Tip 1: How to take a sick leave
Call or write to the employer and inform him that you took the sick leave sheet during the vacation. After the sick leave sheet is closed, take it to the accounting department or send it by registered mail with a notification. Attach to the temporary disability sheet an application to extend your vacation due to illness.
The sick leave sheet must be issued in accordance with the rules put into effect on July 1, 2011, and have all seals and stamps. In case there are corrections (not more than 2), each of them must be certified by the signature of the attending physician and the seal of the institution (polyclinic, hospital), and be accompanied by the inscription "Corrected believe."
The employer must calculate the number of calendar days that occurred during the period of the illness, excluding days that are lawful by law, and issue an order to extend leave. The order should specify the reason for the extension of leave - the sick leave sheet and its number, as well as the date of the beginning and end of the illness. In addition, the person responsible for filling out the work sheet must correct the letters "OT" ("leave") in "B" ("illness") for the entire duration of the sick-list.
If you decide to postpone relying in this case, you leave for another time, write an application to the name of the head of your organization. The head must issue an order to make changes in the schedule of holidays, and the personnel department employee - make an appropriate entry in your personal card.
If you were at your own volition on leave without content, then the employer has the right to refuse to transfer or extend your vacation due to illness. In addition, the sick leave you take during the leave to care for a child or other close relative will not also be sufficient grounds for his extension and transfer.
