Tip 1: How to fill out a statement
The first section of the report is intended for payers of the unified social tax. In the tables, indicate all social insurance costs.
These include: expenses for compulsory social insurance, amounts for social insurance, which are counted as a cumulative result from the beginning of the year, data on the number and cost of permits purchased at the expense of the FSS and payments to affected persons, according to the law.
In four tables of the second section, write down the data on insurance premiums-compulsory social insurance against accidents and occupational diseases at work.
At the end, count the number of injured and disabled days, as well as cases
incapacity for work, due to work-related injuries and other accidents. Specify the total number of calendar days.
Sign the report from the head and chief accountant, be sure to specify the phone number of the performer.
Data of the report should be indicated, according to the current legislation. All data must be clear, inaccuracies are not allowed.
In the address part of the report, the full name of the insured or the full name for the individual should be indicated. The double surname must be indicated via a hyphen. Each indicator is written in a separate column, if there are no indicators, put a dash in the corresponding line.
