Tip 1: How to fill out the statement
The first section of the report is intended for payers of the unified social tax. In the tables list all social insurance expenses.

In the end, count the number of injured and disabled days, as well as the cases
disability due to work injuries and other accidents. Specify the total number of calendar days.
Sign a report with the manager and chief accountant, be sure to include the phone numbers of the contractor.
Report data must be specified in accordance with applicable law. All data should be clear, inaccuracies are not allowed.
In the address part of the report should indicate the full name of the policyholder or name for the individual. The double surname must be indicated with a hyphen. Write each indicator in a separate column. If there are no indicators, put a dash in the corresponding line.
